Faith leaders, community organizations, elected leaders, and residents have been engaging with hospitals and federally qualified health centers (FQHCs) throughout the South Side to collaborate on a new South Side health transformation plan. The goal is a community-driven process, rooted in health equity, that connects existing healthcare organizations and maximizes existing assets, increases access to quality care, and addresses social determinants of health.

The South Side Health Transformation Project has engaged more than 900 South Side residents, healthcare providers, faith leaders, elected leaders, patients and more in an effort to gather input and ideas on what makes a healthy community.
